Acupuncture practice ter Horst handles your personal and medical data with care, in accordance with the General Data Protection Regulation (GDPR) and the Dutch Medical Treatment Agreement Act (WGBO).

Your treatment file

To provide good care, your practitioner is legally required (WGBO) to maintain a file. Your file contains:

Personal details (name, address, date of birth, e-mail address)

Notes about your health condition and complaints

Data on examinations and treatments performed

Relevant lifestyle information (living/work situation, family) where necessary for treatment

Information from your GP or other healthcare providers — only with your explicit consent

Access to your file

Your file is accessible exclusively to:

Your practitioner (duty of confidentiality)

An intern — only with your prior consent

A locum during your practitioner’s absence — only with your consent

A limited portion of your data (name, address, date of birth, e-mail) may be shared with an authorised person for financial administration purposes.

Retention period

Your file is retained for 15 years after the last treatment, in accordance with the statutory retention period under the WGBO. After this period, your data will be securely destroyed.

Data via the website

Personal data may be provided via the contact form and the introductory form on the website (name, e-mail address, telephone number). These are used solely to respond to your enquiry and are never shared with third parties without your consent.
