4 Minors (<16 years)
Name, address, telephone number and e-mail address of both parents

5 Retention period
15 years after the last modification or addition (in accordance with the WGBO)
6 Other use of your data
Your data may also be used for:
Informing other healthcare providers (with your consent), e.g. when concluding treatment or making a referral
Use by a locum during your practitioner’s absence
Anonymised use during peer consultation or instruction of interns
Anonymised use (with your consent) for scientific research
If your practitioner wishes to use your data for any other purpose, your consent will be requested.
7 Data on the healthcare invoice
The healthcare invoice contains the data required by your health insurer:
Name, address, city
Client number and/or date of birth
Possibly BSN (citizen service number)
Date of treatment
Brief description of treatment
Performance code (e.g. 24104 acupuncture)
Cost of the consultation
8 Your rights
At intake we ask for your consent to record and process your data
We record your consent in your file
We record no more data than is necessary for the stated purpose
You have the right to withdraw given consent*
You have the right to access, rectify, delete and receive your own data (data portability)*
We will draw your attention to our privacy policy and where it can be found

9 Access to file data
The following persons outside the healthcare provider may have access to your data:
Possibly a locum
This person is aware of privacy protection rules and is bound by a duty of confidentiality.
10 Website and social media
Messages you post on our digital guestbook, Facebook page or Google Reviews are publicly accessible
11 Security and data breaches
We take the following measures to protect your data:
Technical — security of files, computers and systems
Organisational — agreements and contracts with involved parties
We have a data breach protocol in accordance with the statutory reporting obligation.
